1. The subject of the contract is the supply of medicines, hereinafter referred to as goods, for the Independent Public

Health Care Facility in Siemiatycze, hereinafter referred to as the Contracting Authority, in the quantity and assortment and with the parameters specified in

Appendix No. 1 to the Terms of Use – Assortment and quantity form. 2. A detailed description of the subject matter of the contract, a list of

required delivery limit parameters listed in paragraph 1 shall contain the assortment and quantity form

together with the description, constitutes Appendices No. 1 to the Terms of Use in the details described parts of packages from 1 to 121 to the offer,

an integral part thereof and subsequently an integral part of the supply contract. The data mentioned in the subject matter of the contract

(parameters) and any indication of the equipment (components) shall be understood as the determination of the parameters of the
