Successive deliveries of medicinal products to the company pharmacy of the Hospital and Outpatient Clinic with the Admission Room of Penitentiary No. 1 in Łódź:

- respiratory system and medicines for diseases of the eyes and ears,

- specialist dressings used in the treatment of wounds

The subject of this contract is "successive deliveries to the pharmacy of the Hospital and Outpatient Clinic with the Admission Room of Penitentiary No. 1 in Łódź of medicinal products in estimated quantities and in the assortment clearly indicated in the tabular lists constituting annexes numbered 1 to 2 to the tender specifications. Appendix No. 1 to the SWZ contains an assortment list concerning package No. 1 of the tender, Annex No. 2 to the SWZ contains an assortment list concerning package No. 2 of the tender. The Ordering Party declares that whenever the Ordering Party used trade names of medicines, medical devices and foodstuffs for particular nutritional uses in the assortment form, it did so in accordance with Article 99(4), (5) of the Public Procurement Law only for the purpose of precisely determining the active substance or composition of active substances (or intended use) contained in the ordered medicines and foodstuffs for particular nutritional uses, or specifying medical devices. The use of these names means only that the Ordering Party demands to offer products containing these active substances or these compositions of active substances as the products whose names have been used and allows the offer of both the same products and equivalent products, which should be understood as identical in terms of the composition of active substances intended for and having an effect.
