1. The subject of the contract is the delivery of a medical imaging kit by X-ray fluroscopy along with the construction of a laboratory and adaptation of rooms for the Blessed Father Jerzy Popiełuszko Provincial Specialist Hospital in Włocławek. A detailed description of the subject of the contract in the scope of the medical imaging kit by X-ray fluoroscopy is included in Annex No. 2 and No. 8 and No. 10 to the SWZ. A detailed description of the subject of the contract is included in the SWZ.

1.The subject of the contract is the delivery of a medical imaging kit by X-ray fluroscopy along with the construction of a laboratory and adaptation of rooms for the Blessed Father Jerzy Popiełuszko Provincial Specialist Hospital in Włocławek. A detailed description of the subject of the contract for the medical imaging kit by X-ray fluoroscopy is included in Annex 2 and No. 8 and No. 10 to this SWZ. 2. Preparation of executive documentation, construction, delivery, assembly, installation and commissioning of the angiography laboratory with all components, equipment, accessories, auxiliary and cooperating devices, software along with training and warranty service – will take place in accordance with the technical and functional parameters constituting Annex 2, 8 and 10 to the SWZ. As part of the subject of the contract, the Contractor is obliged to prepare construction and executive design documentation for the adaptation of the rooms of the angiography laboratory, in accordance with the Functional and Utility Program constituting Annex 8 to the SWZ and the building permit and construction project, constituting Annex 10 to SWZ.2.1 The Contractor will be obliged, m.in. to:1) verify the concept of the Angiography Laboratory resulting from the PFU within 7 days of signing the contract and present within this period, the Ordering Party will receive the results (in writing or by e-mail) of this verification, including possible comments,2) preparation of multi-branch design documentation in accordance with applicable regulations and including in particular: a. construction and executive design in all industries (construction and construction, electrical, teletechnical, sanitary, fire protection, road with greenery and all others resulting from the provisions regarding the functioning and design of this type of plants, in particular:- technical specifications for the execution and acceptance of construction works, - technology design - jointly and hereinafter referred to as "documentation".2.2The contractor is obliged to prepare the documentation in a complete state from the point of view of the purpose it is to serve and in accordance with applicable regulations and standards. The documentation will be prepared in accordance with applicable norms, standards and applicable laws, in particular in accordance with:1) the provisions of the Act of 7 July 1994 Construction Law (i.e. Journal of Laws 2021.2351, as amended),2) the provisions of the Public Procurement Law (i.e. z.U.2022.1710, as amended), in particular Articles 100 – 103 of this Act, 3) the provisions of the Regulation of the Minister of Development and Technology of 20 December 2021 on determining the methods and bases for preparing investor's cost estimate, calculation of planned costs of design works and planned costs of construction works specified in the functional and utility program (Journal of Laws 2021.2458),4)regulations of the Minister of Development and Technology of 20 December 2021 on the detailed scope and form of design documentation, technical specifications for the execution and acceptance of construction works and functional and utility program (Journal of Laws 2021.2454),5) guidelines contained in thematic detailed regulations,6) applicable Polish Standards.2.3The documentation is to describe in particular: technology of works, materials and devices, as well as technical and functional parameters of the adopted material solutions, selected technology, devices and equipment. The documentation should describe the subject of the contract using technical and quality features, while complying with Polish Standards transposing European harmonized standards, including all necessary opinions, agreements and verification of design solutions in the scope resulting from the regulations, in accordance with the submitted Functional and Utility Program.2.4The documentation will take into account the provisions of safety and health protection rules in the construction process.2.52.5 All materials provided for in project to perform the subject of the contract must be consistent (...)
