3. Attention!
The Contracting Authority shall inform the contractors of the local vision at the site of the installation of the tomograph, in which the presence is a necessary condition.
Where the Contractor does not participate in the vision, his offer will be rejected under Article 226 (1) (18) of the Law p.z.p.
In order to participate in the local vision, contractors are required to report to the person entitled to contact the contractors, the names of the persons who will participate in the local vision at the latest 2 working days before the specified date to the e-mail address: molenda.m@powiat.kielce.pl
Local vision will be carried out on 29.6.2021 at 12.00.
Please note that arrivals are required to have an identity card with them. The purpose of the local vision is to familiarize yourself with the technical condition of the premises, the electrical connection and establish the scope of the necessary adaptive works.
4. Supplied equipment must comply with applicable standards and shall have the required documents proving the authorisation of the object of the contract for marketing and use in Poland in accordance with the applicable EU Directives and in accordance with the requirements of the Act of 20 May 2010 on Medical Devices ( i.e. OJ 2020 item 186 as amended) and CE Certificates.
5. In the course of the subject of the contract, the Contractor shall be obliged, among others:
1) perform an overload expert of the ceiling, the room in which the tomograph will be installed and the transmission of the original to the Ordering Party.
2) the delivery of a tomograph with the parameters specified in the OPZ;
3) carry out the work of adapting the premises, delivery, setting and commissioning the object of the order in the place indicated by the Ordering Party using the necessary materials, parts and equipment;
4) in the case of reconstruction of the building, obtaining a construction permit;
5) obtaining the required Polish law of the consents for the use of the device;
6) carry out acceptance and specialized tests during the warranty period of the device;
7) verify the correctness of the operation of the equipment with the participation of the Ordering
8) training of Ordering personnel in the field of operation of the tomograph (5 persons min.);
9. granting the guarantee with the provision of documents confirming its granting;

7. If, in the description of the subject of the contract, the Contracting Party indicated trademarks, patents or origin as well as standards provided by a particular Contractor, in this respect the Contracting Authority allows the possibility of using products, equivalent devices, but with technically - qualitative parameters no worse than those indicated or equivalent to the products listed in the description of the object of the order. Furthermore, the technical cooperation of the counterparts offered must be collision-free and maintained at a level not lower than the cooperation of the products mentioned in this description. The Contractor who provides for the use of products equivalent to those described by the Contracting Authority shall demonstrate that the equipment offered is equivalent as they meet the requirements laid down by the Contracting Authority. The burden of demonstrating this conformity lies with the Contractor. In order to assess the equivalence offered by the Contractor of the devices, the Contractor shall attach their description with an indication of the characteristics of equivalence.
8. The item of the order was not divided into parts. The Contracting Authority shall not allow the submission of partial tenders.
9. The Contracting Authority shall not permit the submission of variant tenders and in the form of electronic catalogs.
10. The Contracting Authority does not provide for the award of contracts referred to in Article 214 (1) (8) p.z.p.
