3.1. The subject of the order is the design, delivery and assembly together with the launch of 46 solar installations and 173 micro-solar installations as part of the project implementation. "Clean energy from renewable sources for the inhabitants of the Sitkówka-Nowiny and Piekoszów Communes" on the private real estate of the residents of the partner municipalities for households located in Bławatków, Brynica, Gałęzice, Gniewce, Szczukowice, Szczukowskie Górki, Jaworznia, Jeżynów, Julianów, Łosień , Łosienek, Łubno, Micigózd, Piekoszów, Podzamcze, Rykoszyn, Skałka (part of the town of Gałęzice), Wincentów, Wincentów-Zręby of the commune of Piekoszów and in the villages of Nowiny, Sitkówka, Zowiska, Trzcianki, Kowala Mała (part of Kowala), Kowala, Bolechowice, Zawada (part of the town of Szewce), Szewce, Zgórsko (part of the town of Z plain) of the commune of Sitkówka-Nowiny, divided into four parts.

PART I - design, delivery, assembly and commissioning of 19 solar installations (solar collectors) for the Sitkówka-Nowiny Commune: preparation of design documentation for solar installations to be carried out on buildings or land - belonging to the inhabitants of partner communes, as well as delivery of installation elements and assembly on site solar installations based on completed design documentation, including commissioning and instruction. Three types of solar installations are envisaged, depending on their power and volume of storage heaters (sets), in quantities: Set I: 2 flat solar collectors with a capacity of min. 3166 W, feeding the DHW cylinder with a minimum capacity of 240 dm3 in an amount of 9 pcs. Set II: 3 flat solar collectors with a capacity of min. 4749 W, feeding the DHW cylinder with a minimum capacity of 270 dm3 in the amount of 7 pcs. Set III: 4 flat solar collectors with a capacity of min. 6332 W, feeding the DHW cylinder with a minimum capacity of 370 dm3 in quantity of 3 pcs. A detailed description of the subject of the contract as well as a description of how to perform part of the contract is in Part III ToR entitled: Functional and Utility Program (PFU).

8. List of declarations or documents confirming the fulfillment of the conditions for participation in the procedure and the lack of grounds for exclusion. To confirm the conditions for participation in the procedure and the lack of grounds for exclusion, the contractors shall submit: 8.1. Together with the offer, the JEDZ statement in electronic form with a qualified electronic signature to confirm the conditions in advance participation in the proceedings and no grounds for exclusion. Statements of entities submitting the offer jointly and entities sharing the potential submitted on the JEDZ form should be in the form of an electronic document, signed by a qualified electronic signature by each of them appropriate to the extent that they confirm the circumstances referred to in art. 22 (1) of the PZP Act, i.e. meeting the conditions for participation in the procedure and no grounds for exclusion to the extent that each of the contractors shows that they meet the conditions for participation in the procedure and no grounds for exclusion. 8.1.1. The Employer allows in particular the following format of sent data: .pdf, .doc, .docx, .xps, .odt, rtf. 8.1.2. The contractor fills in EAT, creating an electronic document. Can use the ESPD tool or other available tools or software that allow you to fill in the EAT and create an electronic document, in particular in one of the abovementioned formats. Instructions for completing the JEDZ form are available on the Public Procurement Office website https://www.uzp.gov.pl/baza-wiedzy/jednolity-europejski-dokument-zamycji. 8.1.3. After the contractor has created or generated the EAT electronic document, the contractor shall sign the abovementioned a document with a qualified electronic signature issued by a qualified trust service provider that is an entity providing certification services - an electronic signature that meets the security requirements set out in the Act.

8.1.4. The contractor is required to complete the EAT form in the scope of: Part II, sections: A, B, C, D; Part III sections: A, B, C, D; Part IV section: α; Part VI - signature. NOTE: Instructions for completing the Single European Order Document are set out in Annex 2 to the IDW. 8.2. Within 3 days of posting on the website the information referred to in art. 86 section 5 of the Public Procurement Law, the Contractor shall provide the Ordering Party via the Platform, without additional request, with a declaration of belonging or non-belonging to the same capital group as referred to in art. 24 paragraph 1 point 23 of the Public Procurement Law in order to demonstrate the lack of grounds for exclusion from the public procurement procedure in circumstances ... continued in further sections

Three types of solar installations are envisaged, depending on their power and volume of storage heaters (sets), in quantities: Set I: 2 flat solar collectors with a capacity of min. 3166 W, feeding the DHW cylinder with a minimum capacity of 240 dm3 in an amount of 6 pcs. Set II: 3 flat solar collectors with a capacity of min. 4749 W, feeding the DHW cylinder with a minimum capacity of 270 dm3 in the amount of 19 pcs. Set III: 4 flat solar collectors with a capacity of min. 6332 W, feeding the DHW cylinder with a minimum capacity of 370 dm3 in quantity of 2 pcs. 8. List of declarations or documents - continuation from section II.2.4) part AND:

8.2 .... referred to in art. 24 paragraph 1 point 23) of the PZP Act, according to template No. 4 to IDW. Along with submitting a declaration of belonging to the same capital group as referred to in art. 24 paragraph 1 point 23 of the Public Procurement Law, the contractor should provide, under pain of exclusion from the contract award procedure, evidence that links with another contractor do not lead to distortion of competition in the contract award procedure. SIWZ information from the bank or cooperative savings and credit union confirming the amount of financial resources or the contractor's creditworthiness, issued not earlier than 1 month before the deadline for submitting bids; the document should be provided in the original in the form of an electronic document or an electronic copy of the document certified as being true to the original. 8.3.2. To confirm compliance with the condition referred to in point 5.2.3.1. A statement from the relevant organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund or other document confirming that the Contractor is not in arrears with payment of social security or health insurance contributions, issued not earlier than 3 months before the deadline for submitting bids or another document confirming that the contractor has entered into an agreement with the competent authority on the repayment of these receivables together with any interest or fines, in particular, he obtained the exemption provided for by law, deferment or installment of overdue payments or withholding in full the execution of the decision of the competent authority.
