Procurement of a digital, web-based platform for a needs-based placement of patients of all clinics and facilities of the UKHD to post-care facilities of the health system within the framework of the statutory discharge and transfer management, hereinafter referred to as "discharge management".

The subject of this award procedure is the procurement of a digital, web-based platform for a needs-based placement of patients of all clinics and facilities of the UKHD to post-care facilities of the health system within the framework of the statutory discharge and transfer management, hereinafter referred to as "discharge management".

The procedure must be eligible under KHZG.

The primary goal is to use this digital support to:

- To optimize the placement of a place at the after-care facilities by means of inquiries from qualified subcontractors - in terms of time and content through suitable and op-timized matching procedures

- Automate and ensure the associated documentation of these mediation requests and processes

- To optimize the electronic data exchange of data exchange with the downstream equipment

Thus, the secondary objectives in the context of the implementation of efficient discharge management are:

faster and more economical evaluation of follow-up care to improve the timing of the delay/ digital and transparent mapping of the complete process for follow-up care/ standardized procedure/ ensuring patients' freedom of choice/ effective and legally compliant data protection of patient data/? Relief of employees and relatives in the dismissal organization / Traceability of already initiated measures / Quality assurance / Reporting (MDK)

The digital tool must be available via a web-based interface to all laying parts of the UKHD. These are located both in the administrative areas of the wards, outpatient clinics, as well as in the social services.

The UKHD wants to use this tool to reach as many institutions of the types of care as possible nationwide, such as:

Rehabilitation (AHB FrüReha PhaseB+C,Akutgeriatrie

Home care (Amb. nursing service, intensive care, SAPV, everyday support)

Inpatient care (KZP, day night care, intensive, preventive care, nursing home, hospice, palliative, SAPV)

With regard to data exchange, the following must be taken into account:

- The data exchange must be structured and standardized

- To pledge data storage 341$ SGB compliant

- Based on KHZG criteria, therefore eligible for funding

- Connection to MDK and cost object must be assured

- Connection to ISH/ISH.med of the hospital is to be realized

The service to be provided is to be provided in the sense of a contract for work. The UKHD proposes the EVB-IT contract type Cloud as a basis.

Contract duration: initially 24 months. An extension of 2 years is optional.

The offered solution must be introduced by means of a corresponding project planning, which goes hand in hand with the initialization phase with customizing and introduction of the solution, training and instruction of the key users.

For the following 2 years as well as for the options 2 additional years, support services are to be offered.
