1. The subject of the contract is "Delivery and implementation of the Integrated Domain System and Electronic Document Management System".
2. The order is carried out under the project entitled "E-municipalities in the Lublin Functional Area" (project number: RPLU.02.02.00-06-0001 / 17) co-financed by the European Regional Development Fund under the Regional Operational Program of the Lublin Voivodship for the years 2014-2020, Priority Axis 2 Digital Lubelskie , Measure 2.2 Digitization of the Lublin Functional Area as part of Integrated Territorial Investments.
3. Employer in accordance with art. 36aa section 1 of the PPL Act, partial offers may be submitted, divided into two parts, as follows:
1) part 1 - delivery and implementation of the Integrated Domain System,
2) part 2 - delivery and implementation of an Electronic Document Management System.
1. The subject of lot 1 of the contract is the delivery and implementation of the Integrated Domain System, including, inter alia:
1) pre-implementation analysis,
2) purchase and delivery of perpetual licenses together with ZSD software,
3) implementation of ZSD,
4) conducting trainings for the staff of the municipalities of Niedrzwica Duża, Jastków and Strzyżewice and their organizational units on handling the ZSD,
5) data migration from the currently used software to the new system,
6) delivery of 2 scanners for scanning correspondence (1 for the Niedrzwica Duży commune and 1 for the Jastków commune),
7) providing a free 5-year quality guarantee after completing the system implementation (providing warranty and maintenance services - technical assistance in the scope of the subject of the contract).
2. A detailed description of the subject of the contract is included in Annex 1a to the ToR.

4. Employer, pursuant to art. 29 paragraph 3a of the Public Procurement Law, specifies the following requirements for the employment by the Contractor or subcontractor on the basis of an employment contract of persons performing activities indicated by the Employer in the field of contract performance, the performance of which consists of performing work in the manner specified in art. 22 § 1 of the Act of 26 June 1974, Labor Code (consolidated text, Journal of Laws of 2019, item 1040):
1) the type of activities necessary to perform the contract, which relate to the employment requirements under the employment contract by the Contractor or subcontractor, persons performing activities during the contract: activities directly related to the implementation of the contract, i.e. related to the analysis of IT systems, creation / adaptation of software, system implementation (the requirement does not apply to persons running their own business activity),
2) the method of documenting the employment of persons referred to in art. 29 paragraph 3a of the Public Procurement Law - the Contractor, within 5 days from the date of signing the contract, shall submit to the Employer a statement that persons performing activities directly related to the performance of the contract are employed, under the employment contract, both by the Contractor and subcontractors,
3) the Awarding Entity's powers to control compliance by the Contractor with the requirements referred to in art. 29 paragraph 3a of the Public Procurement Law and sanctions for failure to comply with these requirements: during the contract period, the Employer has the right to submit an application to the labor inspectorate for an inspection. Failure to meet the requirements referred to in item 1 results in sanctions specified in § 8 of Annex 6a to the ToR (regarding part 1 of the contract) and in § 8 of Annex 6b to the ToR (regarding part 2 of the contract). The contracting authority reserves the right to request, at the stage of performing the contract, a statement by the contractor or subcontractor about the employment of a given employee, containing information necessary to verify employment under an employment contract, in particular the name of the employed employee, date of the employment contract, type of employment contract and employee responsibilities.
1. The subject of Lot 2 of the contract is the purchase, delivery, installation and implementation of an Electronic Document Management System (EZD) and the e-Services Portal, including:
1) pre-implementation analysis,
2) purchase and delivery of perpetual licenses together with EZD software,
3) implementation of EZD,
4) system configuration on the computer equipment indicated by the Employer,
5) data migration from the currently used software to the new system,
6) conducting trainings for the staff of Jabłonna Commune and its organizational units on handling EZD,
7) delivery of the required licenses authorizing the Ordering Party to use the System legally,
8) creation and launch of the e-Services Portal (min. 10 e-services indicated by the Employer),
9) delivery of a scanner for scanning correspondence,
10) providing a free 5-year quality guarantee after completing the system implementation (providing warranty and maintenance services - technical assistance in the scope of the subject of the contract).
