The assisted discharge service (ADS) minimises the risk of people being unnecessarily admitted to or delayed in leaving hospital to the detriment of their well being and independence and the flow of the system. This service assists frail and vulnerable people home from their home to hospital, whether they were inpatients or attended A&E.

Assisted patient transport/discharge services will be provided to a number of hospitals trust across the regions.
