An Acute Home Visiting Service provides support to General Practice in delivering quality care through Proactive General Practice by centralising aspects of daily provision. The initial patient contact remains within General Practice through a triage and referral system, maintaining continuity for patients. This service will provide optimal care for patients keeping them at home wherever possible, maximise efficiencies of workforce, and demonstrate value for money by reducing non-elective admissions.
The service will provide a team of appropriately qualified practitioners (who are able to manage the conditions outlined within the inclusion criteria and are independent prescribers) to undertake a same day home visiting service on behalf of GP Practices, in order to assess acutely unwell patients who are deemed at risk of hospital attendance/admission.
