The subject of the contract for this tender procedure is the purchase of medical equipment for the Clinical Department of Ophthalmology with a section for children (hereinafter referred to as the goods), in accordance with the type of assortment and quantities specified in the "Assortment and Price Form" constituting Appendix No. 2 to the Terms of Reference and in accordance with the Offer Form, constituting Appendix No. 1 to the Terms of Reference and meeting the limit parameters specified in the Annex "Technical parameters" (Appendix 1a1, 1a2, 1a3, 1a4 to the Offer Form). A detailed description of the subject matter of the contract can be found in Appendix 2 to the Terms of Reference ("Assortment and Price Form") and in the appendix to the Offer Form ("Technical Parameters").
