Subject of order is the insurance service of the Bytom Commune (Municipal Office and its subsidiaries) divided into 3 parts: Part 1 - Bytom Commune - covers the insurance service of the Bytom Commune (Municipal Office and org. and dependent Communes) Part 2 – Bytomskie Flats - includes the insurance service of the budget unit Bytomskie Flats Part 3 - Motor insurance Employer in connection of Article 34 (5) of the Public Procurement Law provides for the option right consisting in the possibility of increasing the scope of nin.zam. up to 10%. Detailed description of each part of the item includes the attachment to the Terms of Reference. Method of implementing the order item The contractor is required to employ under the employment contract all persons performing work on administrative support of the insurance, i.e. at: issuing policies and other documents insurance, issuing annexes and other documents confirming the introduction of changes in insurance, excluding persons conducting business activity.
1) Part 1 - Bytom Commune - covers the insurance service of the Bytom Commune (City Hall as well as organizational and dependent units of the Commune) in the scope of:
a) insurance of property against fire and other random events,
b) property insurance against burglary and robbery,
c) insurance of electronic equipment against all risks,
d) accident insurance,
e) liability insurance,
Employer in connection with art. 34 section 5 of the PPL Act provides for the option right consisting in the possibility of extending the scope of the nineteenth contract to 10%.
In accordance with art. 13 section 1 and 2 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016. on the protection of individuals with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46 / EC (General Data Protection Regulation) (Official Journal EU L 119 of 05/04/2016, p. 1), hereinafter "GDPR", the contracting authority informs that:

3) Your personal data will be processed on the basis of art. 6 clause 1 lit. c GDPR for the purpose related to this public procurement procedure (and in the case of choosing the most advantageous bid and concluding the contract, also for the purpose related to the performance of the contract);
4) The recipients of your personal data will be persons or entities to whom documentation of proceedings based on art. 8 and art. 96 section 3 of the Act of January 29, 2004. - Public procurement law, hereinafter "PPL Act"
5) Your personal data will be stored until the documentation of the procedure is destroyed in accordance with applicable law or in accordance with the project co-financing agreement, but not less than for a period of 4 years from the date of termination of the public procurement procedure, and if the duration of the contract exceeds 4 years, the storage period covers the entire duration of the contract (in accordance with Article 97 (1) of the Public Procurement Law).
6) The obligation for you to provide personal data directly related to you is a statutory requirement specified in the provisions of the Public Procurement Law related to participation in the public procurement procedure. The consequences of not providing specific data result from the Public Procurement Law

Part 2 - Bytomskie Homes - covers the insurance service of the budget unit of Bytomskie Homes in the scope of:
a) insurance of property against fire and other random events,
b) property insurance against burglary and robbery,
c) insurance of electronic equipment against all risks,
d) property liability insurance.
