                        Deliveries of cytostatics for the Multispecialist Hospital Dr. L. Błażek in Inowrocław 
 1. The subject of the order is the delivery of cytostatic drugs for the Multispecialist Hospital in Inowrocław. 
 2. A detailed description of the subject of the contract is Annex 2 to the ToR.

 a) The cytostatic drugs offered must comply with the Catalog of reimbursed drugs in chemotherapy and drug programs - in accordance with the announcement of the Minister of Health issued pursuant to art. 37 section 1 of the Act of 12.5.2011 on reimbursement of medicines, foods for particular nutritional uses and medical devices (Journal of Laws No. 122, item 696, as amended), 
 b) The Awarding Entity requires that cytostatic drugs with the same international name, in different doses, come from the same manufacturer, 
 c) The Awarding Entity reserves the right to flexibly choose the doses in the scope described in Annex 2 to the ToR. 
 d) The Employer requires that on each invoice with cytotoxic drugs, the Seller provide the EAN code or another code corresponding to the EAN code d
