The subject of the contract is the maintenance of street green areas and summer and winter maintenance of pavements in Zabrze divided into 2 parts:
- part 1: maintenance of street green areas and summer and winter maintenance of pavements in Zabrze: plot Helenka, plot Rokitnica, plot Grzybowice,
- part 5: maintenance of street green areas and summer and winter maintenance of pavements in Zabrze: the plot of Pawłów, the plot of Zabór and DTŚ.
A detailed description of the subject of the contract, the scope and conditions of the works are included in Annex 12 to the ToR - a detailed description of the subject of the contract and a model contract - Annex 4 to the ToR.
The subject of the order is the conservation of street green areas and summer and winter maintenance of pavements in Zabrze: plot Helenka, plot Rokitnica, plot Grzybowice.
A detailed description of the subject of the contract, the scope and conditions of the works are included in Annex 12 to the ToR - a detailed description of the subject of the contract and a model contract - Annex 4 to the ToR. The contracting authority envisages the award of contracts referred to in art. 67 section 1 point 6 of the PPL Act consisting in the repetition of similar services specified in point 3.4.1 of the ToR. The contracting authority provides that the value of these services may be up to 50% of the value of the basic contract. The award procedure consisting in the repetition of similar services may be initiated on the basis of a necessity protocol approved by the Employer describing the scope of services and justification of the need for their performance, and after securing the appropriate financial resources, in particular in the case of:
- use of the contractual amount,
- a protracted procedure for the award of another contract under the open procedure.
The award of the contract will be preceded by conducting direct proceedings after negotiations with the Contractor. The contractor will start performing similar services only after the conclusion of the contract, provided that the conditions referred to in art. 67 section 1 point 6 of the PPL Act. Accession by the Contractor to perform the services covered by the necessity report, without prior conclusion of the contract with the Employer does not entitle the Contractor to request the Employer to pay remuneration for the performance / partial performance of services.
The Contractor is obliged, before the deadline for submitting bids, to lodge a security deposit in the amount of: part 1: PLN 8,000.00. In order to pre-confirm the fulfillment by the Contractor of the conditions for participation in the procedure and the lack of grounds for excluding the Contractor from participation in the procedure, the Employer requests:
valid for the day of submission of tenders, the single European procurement document, drawn up in accordance with the standard form template set out in the European Commission implementing regulation issued on the basis of art. 59 clause 2 of Directive 2014/24 / EU, hereinafter referred to as the "single document". The contractor is obliged to submit a uniform document to the extent indicated in Annex 5 to the ToR. The contractor fills in EAT, creating an electronic document, using the available tools. The Contractor may use the ESPD tool, the electronic JED / ESPD tool available on the Public Procurement Office website (link to the tool: http://espd.uzp.gov.pl) or other available tools or software that allow you to complete the EAT and create electronic document. The Purchaser allows in particular the following formats of sent data: .pdf, .doc, .docx, .odt.
After completing the EAT, please save and sign with a qualified electronic signature, then attach to the offer in accordance with the ToR requirements.
The Contractor, which refers to the resources of other entities, in order to demonstrate the absence of grounds for exclusion and compliance with them, to the extent that it refers to their resources, the conditions for participation in the procedure also submits uniform documents regarding these entities.
If Contractors jointly apply for a contract, a single document shall be submitted by each of the Contractors jointly applying for the contract. This document confirms compliance with the conditions for participation in the procedure and the lack of grounds for exclusion to the extent that each of the Contractors shows that they meet the conditions for participation in the procedure and no grounds for exclusion.
