1. The subject of the contract is group life insurance for employees and family members of employees of the Independent Public Complex of Health Care Facilities in Ostrów Mazowiecka.

2. The Contracting Authority does not guarantee that all employees employed and members of the families of employees of the Ordering Party will enter into group life insurance.

3. During the period of insurance, there may be changes in the number of insured persons.

4. If an employee or a family member of an employee resigns from insurance, the Employer is not obliged to pay an insurance premium for them.

5. Detailed terms and conditions of the contract in accordance with Appendix 1 to the Terms of Reference.

6. Professional and age structure of the Contracting Authority's employees in accordance with Appendix No. 2 to the Terms of Reference.
