Our current contracts for laboratory labels, transport bags and request forms are coming to an end and we are looking for a provider(s) to continue to supply this service. Our main objectives are as follows:

— A service which enables us to continue to meet our current CPA / ISO requirements for our sample transport and labelling;

— Compliance with UN regulations for specimen transport;

— Best Value for money.

Pathology sample bag to be able to transport Pathology request forms and samples as a secondary line of protection against spillage. The purpose of this bag is to contain any leaks if an individual bag is compromised.

Estimated supply: 80 000 bags per annum. This is not a commitment to purchase.

This lot is to include 2 designs. These are referred to as ‘Pathology Request Forms’ and ‘Blood Bank Request Forms’. We would expect that suppliers would provide both designs of form.

To produce a barcoded label suitable for application to the Pathology request form and to produce primary tube labels to be read on DBH Analysers.
