Hosting and operation of the BD-online application by an external IT partner for KV Nordrhein

The medical emergency service or on-call service of the statutory health insurance physician serves the medical care of patients in the area of responsibility of KV Nordrhein outside the opening hours of the doctors' practices of the resident doctors. The emergency service only includes medical services for illnesses that are not acutely life-threatening. Dealing with acutely life-threatening illnesses is the responsibility of the rescue services of the fire brigades. KV Nordrhein is not involved in this emergency care.

The emergency medical service can be used by patients nationwide under the uniform telephone number 116117 and is provided by means of various measures organized by KV Nordrhein.

On the one hand, KV Nordrhein operates about 40 emergency practices, often in close proximity to the outpatient clinics of the hospitals, in order to relieve them and to be able to be easily visited by patients. On the other hand, there are doctors who provide emergency services, either in their own practices or in the form of home visits.

For the preparation and implementation of the emergency services, KV Nordrhein uses the "BD online" application from Medlinq GmbH.

The BD online application is used to prepare and manage the rosters of the doctors who provide emergency services. The system is automatically supplied with the master data of all doctors in the area of responsibility of KV Nordrhein from the master data systems of KV Nordrhein. On the basis of the doctor's master data, the district offices of the KV Nordrhein propose duty rosters that the doctors influence independently via an included web application, either by accepting or rejecting proposed services or by exchanging the planned services with each other by means of an integrated exchange platform. The result of the use of BD online is a duty roster, often supplemented at short notice, which is continuously passed on to the ELS 3 operations control system via an interface. BD online is a web-based application based on Ruby on Rails and mySQL (MariaDB) that must be run on Linux (Ubuntu 18.04 LTS).

For secure, highly available operation of the application, it should be outsourced to an external partner who will take over both hosting and application operation.

System operation requirements

In addition to providing the data center and the infrastructure for the software applications, the IT partner is responsible for the system operation in its entirety. The operation of the applications for the emergency medical service must be carried out according to standard IT operating processes. The IT partner must therefore already use appropriate and certified ITSM processes according to ITIL/Cobit. To systemically support the ITSM processes of the IT partner, the IT partner has already introduced a corresponding ITSM software solution and uses it accordingly.
