Federal policy support for the development and implementation of an interfederal plan for integrated care based on the Quintuple Aim (5AIM) and for supporting, at the meso level, the process of change towards integrated care in Belgium

This contract concerns the support and strengthening of the teams of the federal administrations, on the one hand to define and deepen a strategy of change towards integrated care at the macro, meso and micro levels, and on the other hand the support of actors at the meso level to ensure, through strong steering, the development and organization of care integrated for the population in a defined territory.
