Purchase, delivery and assembly of a CT scanner as part of the implementation of the project entitled "Increasing the efficiency of diagnosis and treatment of civilization diseases, including cancer, by purchasing a new generation CT in SP ZOZ in Głubczyce."

A detailed description of the subject of the contract is attached as Annex 2 to the ToR posted on the website of the Employer: www.zozglubczyce.biuletyn.info.pl, which is an integral part of the ToR.

The subject of the order includes delivery to the headquarters of the Employer of a computer tomograph with technical parameters specified in Annex 2 to the ToR.

The subject of the contract also includes:

a) carrying out adaptation works for rooms and technical infrastructure (if required for the installation of the offered CT scanner), necessary for the installation, commissioning and proper functioning of the CT scanner laboratory, including:

- restoring the technical condition of the workshop after the work has been carried out (e.g. filling holes in the floor covering if damaged),

- design of permanent shields for the CT workshop (if required),

- installation of the apparatus, foundation of the apparatus and approval of the shield design in the WSEZ, locally appropriate in the Contractor's bid price (if required) within the offered bid price,

- acceptance tests and after installation of the device within the offered offer price,

- performing specialist tests during the warranty period in accordance with the Regulation of the Minister of Health of 25.8.2005 on the safe use of ionizing radiation for all types of medical exposure;

b) assembly, installation and commissioning of the device together with all necessary tests and checks;

c) software implementation, installation and configuration;

d) training the Employer's staff in the operation and maintenance of a computer tomograph at the Employer's headquarters, on dates agreed with it;

e) providing a 24-month warranty on all elements, parts and modules of the TK apparatus, software, equipment as well as construction and installation works, counted from the date of signing the acceptance protocol confirming the commissioning of the apparatus.
