1. The subject of the contract is the provision of voluntary health insurance services to employees of the Ordering Party and persons employed under other civil and legal contracts, hereinafter referred to as “Entitlees” and their family members in the scope of the following packages:
1) Basic Package,
2) Extended Package,
3) Package Full,
4) VIP package,
5) Rehabilitation Package.
2. The contracting authority shall be jointly acting entities - i.e.:
- PKP S.A.
- PKP Polskie Railway Linie S.A.,
- PKP Cargo S.A.,
- PKP Intercity S.A.,
- CS Natura Tour Sp. z o.o.,
- PKP Intercity Remtrak Sp. z o.o.,
- Foundation of the PKP Group,
- PKP Cargotabor Sp. z o. o.,
- Cargotor Sp. z o. o.,
- PKP Telkol Sp. z o. o.
on behalf of and for which the Leader of Procedure acts: PKP S.A.
The subject of the contract is the provision of voluntary health insurance services for employees of the Contracting Authority and persons employed under other civil and legal contracts, hereinafter referred to as “Entitlees” and their family members in the scope of the following packages:
1) Basic Package,
2) Extended Package,
3) Package Full,
4) VIP package,
5) Rehabilitation Package.
2. A detailed description of the Subject of the Order (hereinafter “OPZ”) is set out in Annex 1 to the SWZ.
3. Package numbers are indicated in price tables A, B and C contained in the Offer Form, which is Annex 2 to SWZ. These are estimated quantities and serve the Ordering Party only to compare offers. The actual number of packages will depend on the current needs of the Ordering Party.
4. Approximately 88.20% of the estimated total number of packages will be subsidised by the Contracting Entities.
5. The Contracting Authority allows the possibility to entrust the execution of part of the contract to subcontractors
6. The Contractor is obliged to indicate in the declaration JEDZ, the model of which is Annex 3 to the SWZ, and the tender form, the model of which is Annex 2 to the SWZ, the parts of the contract to which it intends to entrust to subcontractors and specify the names of the subcontractors (as far as they are known in the present stage).
