The subject of the contract is the purchase and supply of food, divided into lots:
Part 1 - red meat, red meat sausages, animal fats;
Part 2 - poultry meat, poultry meat;
The type, as well as the quality requirements of the ordered assortment are included in the Price Form – Annex 3a (for part 1) and 3b (for part 2) to the SWZ and the Description of the Subject of the Order – Annex 5 to the SWZ (for parts 1 and 2).
Part 1 - red meat, red meat sausages, animal fats1. The subject of the contract is the purchase and supply of food, divided into lots:

2. The type, as well as quality requirements of the ordered assortment is included in the Price Form – Annex 3a (for part 1) and 3b (for part 2) to the SWZ and Description of the Subject of the Order – Annex 5 to the SWZ.
3. The quantities of individual assortment items are not binding in the scope of implementation of the provisions of the concluded contract. They serve only as a tool for evaluating and comparing offers and thus for selecting the most advantageous offer and signing a contract with the contractor. The unit prices of the assortment, which will be presented in the offer by the Contractor, are binding. The quantities of assortment specified in the price form, which the Contractor should submit together with the offer, result from historical data in the scope of the Ordering Party's needs met, i.e. orders from previous years and planned nutritional needs for 2024. The final quantity of the ordered goods will depend on the current needs of the Ordering Party and the financial constraint (the amount intended to finance the order). The supply contract shall be concluded with the Contractor up to the gross amount (the amount that the Contracting Authority intends to allocate to finance the basic contract, which the Contracting Authority will make available after the deadline for submitting tenders) and will be carried out at the unit prices included in the Contractor's offer and other provisions contained in the contract template.
4. The delivered goods must meet the requirements contained in the description of the subject of the order. Products must meet the Polish Standards indicated in the OPZ. Products supplied must be fresh, with a shelf life according to applicable standards and be of the first grade. The offered products should meet the sanitary conditions of their acquisition, production, processing, storage, transport and direct sale in accordance with applicable regulations.
5. The Ordering Party does not allow the use of substitute packaging for food products.
6. The quantities of goods and unit prices specified in the price form are determined for the purpose of evaluating offers, while unit prices are additionally used to evaluate the ordered products and the type of assortment purchased
7. Ordering Party's requirements related to the delivery of food products:
7.1. The Contractor shall ensure the delivery of the subject of the order successively according to the assortment specified in the price form;
7.2. deliveries shall take place on dates agreed with the Ordering Party with the frequency specified in the description of the subject of the contract each time, on the basis of the order placed by the Ordering Party. The order can be placed by phone, fax or email;
7.3. The Ordering Party reserves the right to attach to the first delivery of food products the manufacturer/contractor a statement confirming that the food products come from a producer who is under constant supervision of the competent Veterinary Sanitary Inspection. If the products come from an entity other than their manufacturer, the Contractor shall additionally attach a certificate confirming that this entity is under constant supervision of the competent District Sanitary Inspectorate or Veterinary Sanitary Inspection. With regard to the supply of food products, the Contractor shall, at the request of the Contracting Entity, be obliged to present the documents referred to above.
7.4. The Contractor shall deliver the goods to the Ordering Party's warehouse located in:
7.4.1 JW 1158 in Łask at 9-go Maja 95, Gucin 58a and
7.4.2. JW1551 in Sieradz at 78 Wojska Polskiego Street.
8. In the cases specified in the model contract – Annex 4 to the SWZ, at the request of the Ordering Party, deliveries may be carried out within the responsibility of 32 BLT, i.e. 9 poviats of the Łódź Voivodeship – Bełchatów, Łaski, Pabianicki, Pajęczański, Poddębicki, Sieradzki, Wieluński, Wieruszów, Zduńskowolski.
