North East Lincolnshire Clinical Commissioning Group (NEL CCG) hereby invites applications from suitably qualified and experienced providers to provide a dementia support service in North East Lincolnshire for a 3-year period.

The aim of the service will be to deliver a range of accessible, high quality, cost effective services that will promote, support and improve the mental, physical and emotional well-being of all people living with dementia within North East Lincolnshire. The service will provide advice, information and signposting to support people with dementia to participate and continue daily living activities important to them and to make informed choices on how they spend their time, encouraging them to maintain their independence, autonomy and functional abilities for as long as possible.
