1. The subject of the contract is: Supply of equipment for performing procedures: ablation and cryoablation for a period of 12 months. 2. The subject of the order has been divided into parts - 2 packages / tasks. 3. The detailed scope of the contract is specified in Annex No. 1 to the SWZ – price form. Other conditions regarding the performance of the contract are set out in the model contract, constituting Annex 5 and 5A to the SWZ.
