The subject of the contract are successive deliveries of medical devices for spinal procedures for "Poddębice Centrum Zdrowia" Sp. z o.o. in Poddębice, divided into 2 packages in quantities and assortment specified in Annex 2 to SWZ – Assortment and price form. A detailed description of the subject of the contract is specified in the Assortment and Price Form - Annex 2 to the SWZ. The Ordering Party allows the submission of partial offers, i.e. for individual packages, the description of which is included in the Assortment and Price Form, Appendix No. 2 to the SWZ. The contracting authority does not allow bidding for individual items as part of packages. The contractor may submit a bid for one, several or all lots. An order for all parts may be awarded to one – the same Contractor.
