1. The subject of the contract are successive deliveries of Milk and Milk Products in accordance with the requirements of the food law to recipients and to the places of delivery specified in Annex 3 to the contract, with a minimum shelf life specified in OPZ point 2.

2. The detailed quantitative specification is set out in Annex 1A to the SWZ. That annex is also a form for calculating the proposed offer price.

3. A detailed description of the subject of the contract in terms of quality requirements, expiry dates, labelling and packaging are the documents specified in Appendix No. 5 to the Terms of Reference, developed by the Military Research and Implementation Centre of the Food Service in Warsaw. These annexes are an integral part of the description of the subject matter of the contract and this Terms of Reference.

4. There are 4 delivery warehouses for the above-mentioned subject of the order:

- Food warehouse of the Security Group Rzeszów – 5 Lwowska Street, Rzeszów.

- Food warehouse of the Nisko Security Group – 20 Sandomierska Street in Nisko.

- Food warehouse of the Jarosław Security Group – 80 3 Maja Street, Jarosław.

- Food warehouse of the Security Group Przemyśl – 29 Listopada 1 Street, Przemyśl.

5. Deliveries of the subject of the order will take place with a frequency of up to 3 times a week for each of the recipients in the period from Monday to Saturday at the hours agreed with the recipient.
