NHS Stoke on Trent CCG are inviting expressions of interest for the provision of a Dementia Advisory Service.

The Service description:

The Dementia Advisory Service will support people with a diagnosis of dementia and their families/ carers through various ways including:

— Being a first point of contact for information and signposting needs throughout a person's life with dementia and for their carers and families.

— Helping people to navigate and access appropriate services, ensuring that individuals are able to make contact with the right person for help, and supporting them to do so as necessary.

— Providing information to ensure people know of their options, and assisting people to plan for the future.

— Facilitating local connections and access to peer support networks.

— Ensuring that information about local services for people with dementia and their families/carers is recorded, accessible and maintained/ updated.

The aim of the service will be to ensure that people diagnosed with dementia, their families and carers have access to a named point of contact that can support them during the course of the illness, ensuring that they have information about the likely impact of dementia as the illness progresses. The advisors should have knowledge/ information on the range of services and support available in Stoke-on-Trent for people with dementia, their families and carers and how these can be accessed, including making arrangements where necessary for people to access these services and support as their needs change.

All other relevant information including KPI's will be contained within the Service Specification which will be published with the Invitation to Tender documents.

The anticipated start date for the service is no later than 1.4.2014 and therefore the provider will need to be in a position to deliver the service from this date.
