4.1. The subject of the contract is the purchase of private medical care services for doctoral students of doctoral schools of the University of Lodz.

The private medical care service includes group medical care for the above-mentioned doctoral students of the University of Lodz and their family members in the form of a medical subscription in the following packages:

1) individual package - dedicated to doctoral students,

2) partnership package - dedicated to a doctoral student and one person from: spouse / partner / child (regardless of age),

3) family package – dedicated to doctoral students and spouses/partners and children, regardless of their number (up to the age of 18, and if they are educated until the end of education, no longer than until the age of 25).

Each of the packages indicated above will be available in three variants: basic, optimal and extended.
