The service will be based in a defined geographical community location (not an acute hospital site). Its purpose is to provide dermatology services for the case mix outlined below which:
Ensure the provision of a safe and effective Dermatology community based service to NHSWD patients.
Achieve all local and national waiting time targets.
Ensure that all referrals are managed appropriately including feedback to referrers if not in line with the criteria for the service.
Provide an equitable service to all NHSWD patients served by the defined lot.
Ensure equitable, timely and appropriate access to dermatological advice, diagnosis, treatment and follow up in line with the identified level 3 case mix.
Develop local service provision which aims to improve access and choice of provider and location for patients and clinicians.
Ensure practice is clinically effective, efficient and safe environment in line with national and local guidance and standards.
Deliver high quality care that can be measured against patient’s own experience.
The outpatient service will be provided within an appropriate setting(s) within the Wakefield District area.
Improving knowledge of Wakefield District health professionals and patients around the management of skin diseases through the development and implementation of education programmes for GPs, GPwSI and other practitioners on the agreed clinical pathway.
Case Mix for Community Dematology services.
Consultant Led Interventions:
Patients with chronic skin problems previously requiring specialist care e.g. immuno-suppressive treatment for psoriasis, eczema, lupus etc.
Acne requiring isotretinoin / roaccutane.
Basal Cell carcinoma.
Keratoacanthoma.
Cutaneous Horn.
Blistering skin disorders.
GPSI led interventions:
Uncontrolled eczema, psoriasis, urticaria, acne (not requiring isotretinoin / roaccutane).
Undiagnosed or persistent skin rashes.
Actinic Keratosis (severe).
Bowen’s Disease.
Skin lesions without suspicion of malignancy requiring further assessment.
Basal Cell carcinomas (excluding those on or around the nose, mouth, eyes or ears) - [GPwSI accredited in skin surgery only].
