1. The subject of the contract is the supply of reagents together with the lease of integrated analyzers of the biochemical-immune system for the needs of CLA

2. Description of the conditions of conduct:

a) Agreement for a period of 36 months

(b) use of apparatus in the form of leases

(c) two apparatus, i.e. the main and the backup reagent-compatible apparatus, together with two water treatment plants;

d) year of production of the equipment no later than 2021

(e) the supply of reagents, control materials, calibrators and materials from the equipment supplier.

f) integrated apparatus, i.e. in the form of a Platform

3. A detailed description of the subject of the contract is provided by Attachments No. 2 to 2 H to SWZ.

4. The Contractor is obliged to perform the contract on the terms and conditions described in the contract templates constituting Annexes 5 and 6 to the SWZ.

6. The Contracting Party shall not allow the possibility of submitting partial tenders.
