1. The subject of the order is the supply of lenses and consumable materials in cataract operations and vitrectomy.
2. For a detailed description of the item of the order, together with the quantities ordered, see Annex 2 to SWZ (assortment and price form).
3. The conditions for delivery and receipt of the subject of the contract are set out in the draft contract forming Annexes No 3A and 3B to SWZ respectively.
4. The assortment offered should be authorised for marketing and use in the territory of the Republic of Poland and comply with the requirements laid down by the Act of 20 May 2010 on medical devices (i.e. Journal of Laws of 2021 item 1565, as amended).
