Description:
Performing the duties of the Investor's Supervision Inspector for the following task: "Counteracting water erosion in mountain areas - construction of three culverts in the Brzozów Forest District"

The subject of the contract is the service of fulfilling the obligations of the Investor's Supervision Inspector for construction works, which were described in the Description of the Object of the Order (Annex 7 to the ToR) of the investment project called "Counteracting water erosion in mountain areas - construction of three culverts in the Brzozów Forest District" in the Agreement, in accordance with applicable law, technical and construction regulations, principles of technical knowledge, maintaining the highest professional diligence and guidelines for the Operational Program Infrastructure and Environment 2014-2020 under Priority Axis II - Environmental Protection. The procedure is conducted in accordance with the rules provided for . "Reverse procedure" referred to in Article 24aa section 1 and 2 PZP. Pursuant to the provisions cited, the Employer will first evaluate the offers and then examine whether the contractor whose offer has been assessed as the most advantageous is not subject to exclusion and meets the conditions for participation in the progress.

The provision of the investor's supervision service will consist in particular of managing coordination and investor's supervision over construction works entitled: "Counteracting water erosion in mountain areas - construction of three culverts in the Brzozów Forest District". The duties of the supervision inspector include: a) conducting investor supervision within the meaning of provisions referred to in art. 25, 26 of the Act of 7 July 1994 Construction Law (Journal of Laws of 2018, item 1202, as amended) and implementing regulations, b) exercising careful supervision and ensuring that construction works commissioned by the Employer are carried out only by the Contractor and subcontractors or subcontractors specified in the offer specified in the procedure set out in the contract with the Contractor of construction works, c) before commencing work, familiarizing with the documents necessary for the implementation of the subject of the contract, being in the possession of the Employer, d) handing over the construction site with participation Employer, organization and performance of receiving activities (final acceptance, warranty acceptance and post-warranty acceptance), in accordance with the requirements and legal provisions and technical documentation. E) ensuring constant material and quality control of building materials; f) ensuring control of the correct performance of works at an appropriate level health and safety protection g) settlement of the supervised scope of works, checking settlement documents: as-built cost estimate (for a given task) submitted by the Contractor of the investment together with the final settlement of the investment, h) keeping photographic documentation (especially disappearing works) in electronic form throughout the entire investment process and providing it The Employer after the final acceptance of the works, or at each request of the Employer, i) ensuring that the condition of the works and the construction site is secured for a break in the construction process to the extent that events that do not result in damages in relation to the Employer will occur, j) supervision in the duration of the warranty (5 years) and the guarantee granted by the Contractor of construction works, which will be a maximum of 5 years from the date of signing the final acceptance report of works without comments.k) exercising control over the compliance of the investment with the contract, areami and industry standards, principles of technical knowledge, technical specification of execution and acceptance of works and a construction design. Agreeing with the Employer and the person supervising the author of any possible changes and deviations from the original design documentation. L) immediately notifying the Employer of the need for the contractor to perform additional works, supplementing those not provided for in the contract concluded with the Contractor of construction works (without the consent of the Employer, the Contractor is not authorized to issue the Contractor works, orders for additional and supplementary works) and checking and verifying the contractor's cost estimates for complementary works, m) controlling the correctness of keeping the construction log and making entries in it stating all the circumstances relevant to the proper execution of the works, n) participation in warranty and post-warranty inspections; confirmation by the Supervision Inspector of the abovementioned obligations are inspection reports and the final inspection report during the warranty period, which the Purchaser is obliged to carry out no later than two months before the expiry of the warranty period, stating the absence of defects or defects or a report on the removal of defects and defects found during this review, l) performance of the Subject of the Agreement by the person indicated in the offer, who has appropriate professional qualifications, if the Contractor for this person / person has received points in the criterion "Qualifications and experience of persons who will participate in the implementation of the contract." About the planned change of persons or additional persons by whom the Perform Subject of the Agreement, Wyk. Is obliged to notify the Purchaser.
