The subject of the contract is the purchase, delivery, installation and commissioning of the X-ray apparatus with equipment along with the adaptation of rooms for the needs of the Department of Diagnostic Imaging of the Provincial Hospital Complex in Kielce and the provision of maintenance services during the warranty period for the task performed. A detailed description of the subject of the contract is set out in Annex 2 to the SWZ - Description of the subject of the contract. The scope of construction works for the adaptation of rooms and activities necessary for the installation and commissioning of the X-ray apparatus is included in Annex 2a to the SWZ - PFU Functional and Utility Program. Information on the rules for delivery, guarantee and remuneration is included in Annex 3 to the SWZ – Draft provisions of the contract.

The subject of the contract is the purchase, delivery, installation and commissioning of the X-ray apparatus with equipment along with the adaptation of rooms for the needs of the Department of Diagnostic Imaging of the Provincial Hospital Complex in Kielce and the provision of maintenance services during the warranty period for the task performed.

The subject of the contract will be implemented in the following stages:

a. delivery of a brand new, unused, not reconditioned, not exhibition, marked in accordance with applicable regulations of the X-ray apparatus with equipment, the technical parameters of which are specified in Annex 2 to the SWZ - Description of the subject of the contract constituting Annex 1 to the contract - STAGE I

b. dismantling and utilization of the existing X-ray apparatus in the laboratory, development of the necessary design documentation and performance of all works and construction works in order to adapt the rooms of the X-ray laboratory necessary for the assembly of the supplied X-ray apparatus and installation and commissioning of the device in accordance with Annex 2a to the SWZ - PFU Functional and Utility Program constituting Annex 2 to the contract - STAGE II

c. conduct training for technicians in the effective and safe use of the X-ray apparatus within the dates and scope specified in Annex 2 to the SWZ - Description of the subject of the contract constituting Annex 1 to the contract - STAGE III

d. provision of maintenance services for installed systems and devices during the warranty period - stage IV
