1. The subject of the contract is group life insurance for employees and family members of employees of the Independent Public Health Care Institution of the Ministry of the Interior and Administration in Lublin.

2. The Contracting Authority does not guarantee that all employees employed and members of the families of employees of the Ordering Party will enter into group life insurance.

3. During the period of insurance, there may be changes in the number of insured persons.

4. If the employee or family member of the employee resigns from the insurance, the Ordering Party is not obliged to pay the insurance premium for them.
