The subject of the contract are "Purchase and delivery of drugs for the drug program – treatment of patients with cystic fibrosis with an inhaled antibiotic Tobramycin to SZPZOZ im. Children of Warsaw in Dziekanów Leśny.Successive deliveries according to the current needs of the Ordering Party to the Hospital Pharmacy at the expense of the Contractor for 1 package from the date of conclusion of the contract for 12 months. The Ordering Party reserves the right to extend the term of the contract by a maximum period of 6 months

in the event that the assortment or contract value is not fully used during the basic term of the contract.

Deliveries will be made within 48 hours from the date of placing a written order sent by e-mail – this is the maximum deadline for completing the order. If the Contractor proposes a shorter deadline for the contract, it will be obliged under the contract to deliver within the proposed deadline (this is one of the criteria for evaluating offers).
