Recovery and Physical Rehabilitation in Community Nursing Homes Service to be provided by Nursing Homes for people undertaking recovery and rehabilitation in a Nursing Home.
— To provide an active part of an accessible and effective Recovery and Rehabilitation Service to ensure that adult island residents with physical disabilities receive interventions to enable them to achieve increased functional independence.
— To be able to support the recovery and step up/stepdown phases for people who have recently been acutely ill.
— To reduce dependence on health and social care and enable self-management.
— To facilitate interdisciplinary collaboration and an integrated approach to assessment, treatment, care and discharge planning.
This was an Open procedure. Service Commencement is 1.7.2018. To fulfil the requirement to supply 50 beds in total, the Contracting Authority welcomed submissions by lead provider with subcontracting arrangements, a consortium/ joint venture agreement. An extended ITT window was provided for such arrangements to take place. The Contracting Authority also offered to publish a provider's details on the E-Tendering portal for other organisations to see if they wish to be considered as a party member in a subcontracting arrangement if the cannot fulfil the requirement alone. Please note the contract value included the extension period. 3 + 2 years total. Service starts 1.7.2018.
