Description:
Purchase of two type C medical ambulances

The subject of the contract is the purchase and delivery of 2 brand new, type C ambulances with equipment (stretcher and transporter) for the needs of Medical Rescue Teams. A detailed description of the subject of the contract is included in Part III SWZ - OPZ - table of minimum technical and utility requirements for ambulances.

1. The subject of the contract is the delivery of 2 type C ambulances meeting the requirements of the PN: EN 1789 standard with equipment for the needs of the Medical Rescue Team.

2. The offered subject of the contract must have documents allowing it to be used on the road in the Republic of Poland, enabling its registration and operation immediately after acceptance by the Ordering Party in accordance with applicable law, i.e.:

1) comply with the requirements of current standards PN-EN 1789+A2 and PN-EN 1865-1+A1 or equivalent standards to the extent relevant to this procedure;

2) have a valid approval (for a car with body) issued in accordance with the provisions of Section III, Chapter 1a of the Road Traffic Act of 20 June 1997 (i.e. Journal of Laws of 2022, item 988, as amended);

3) meet the conditions set out in the regulations regarding the technical conditions of vehicles and the scope of their necessary equipment in accordance with the Regulation of the Minister of Infrastructure of 31 December 2002 on the technical conditions of vehicles and the scope of their necessary equipment (i.e. Journal of Laws of 2016, item 2022, as amended);

4) meet the conditions set out in the provisions regarding the designation of the State Medical Rescue System in accordance with the Regulation of the Minister of Health of 17 December 2019 on the designation of the State Medical Rescue system and the requirements for uniforms of members of medical rescue teams (Journal of Laws of 2019, item 2487)

5) meet the conditions set out in the Regulation of the Minister of Infrastructure of 31 August 2022 on the detailed activities of authorities in matters related to the admission of a vehicle to traffic and templates of documents in these matters (Journal of Laws of 2022, item 1849)

6) The subject of the contract must be unused, not demonstration, complete and ready for use and have the equipment and parameters indicated in Part III SWZ – i.e. the Description of the Subject of the Contract.

7) The Contractor shall provide an unlimited mileage guarantee for the delivered ambulance, the validity of which will start from the date of signing the positive vehicle acceptance protocol:

(a) for the base vehicle, for a period of 24 months (fixed value not assessed);

b) for medical buildings – for a period of not less than 24 months (assessed value);

c) for varnish coating – for a period of not less than 24 months (assessed value);

d) for the perforation of the bodywork – guarantee for a period of not less than 24 months (assessed value);

e) for medical equipment - stretcher and carrier - for a period of not less than 24 months (assessed value),

8) The Contractor shall provide an unlimited mileage guarantee for the air conditioning system for a period of 36 months (fixed value not assessed) counting from the date of signing the positive acceptance protocol.

9) Upon delivery of the subject of the contract, the Contractor shall provide the Ordering Party in Polish with all documents necessary for use, in particular:

a) operating instructions, including instructions for use for all bodywork components

specialist for the exterior and interior of the vehicle and equipment;

b) warranty conditions (books, cards, technical passports);

(c) a list of consumables used in the day-to-day operation of the ambulance;

d) information on the conditions of service in the post-warranty period;

(e) as required by the provisions in force:

the ambulance certificate of conformity (CoC) issued in accordance with the regulations on the basis of the type-approval certificate held;
