1. The object of the contract is the provision of mobile telephony services along with the supply of mobile phones, modems, accessories with active and configured SIM cards for the Marshal's Office of the Małopolska Voivodship and voivodship self-government organizational units and voivodship legal persons, i.e .:
a) Regional Center for Social Policy in Krakow,
b) Krakow Center of Rehabilitation and Orthopedics in Krakow,
c) Małopolska Traffic Center in Tarnów,
d) Krakow Office of Geodesy and Agricultural Areas in Krakow,
e) the Voivodeship Labor Office in Krakow,
f) the Małopolska Entrepreneurship Center in Krakow,
g) Provincial Roads Board in Krakow,
h) Marshal's Office of the Lesser Poland Voivodship
2. The services will be provided through the use of active SIM cards provided by the Contractor

The object of the contract is the provision of mobile telephony services along with the delivery of mobile phones, modems, accessories with active and configured SIM cards for the Marshal Office of the Małopolska Voivodship and voivodship self-government organizational units and voivodship legal entities, i.e .:
a) Regional Center for Social Policy in Krakow,
b) Krakow Center of Rehabilitation and Orthopedics in Krakow,
c) Małopolska Traffic Center in Tarnów,
d) Krakow Office of Geodesy and Agricultural Areas in Krakow,
e) the Voivodeship Labor Office in Krakow,
f) the Małopolska Entrepreneurship Center in Krakow,
g) Provincial Roads Board in Krakow,
h) Marshal's Office of the Lesser Poland Voivodship,
2. The services will be provided through the use of active SIM cards provided by the Contractor (keeping the previous ordering of the Ordering Party)
In voice tariffs, date and web browser. The service is to provide, among others: voice communication (outgoing and incoming calls), text (SMS), multimedia (MMS) and access to Internet resources.
3. The Awarding Entity anticipates the transfer of individual numbers to a new Contractor after the completion of the existing contracts and the expiration of the obligations arising therefrom.
4. The terms of the contract are contained in the contract template constituting Annex 3 to the ToR.
5. A detailed description of the subject of the order is attached as Annex 4 to the Terms of Reference (being at the same time Annex No. 1 to the contract template).
6. The contract is co-financed from the European Union funds from the European Social Fund under the Regional Operational Program of the Małopolska Voivodship 2014-2020.
7. Designation according to CPV: 64212000-5 - Mobile telephony services, 72400000-4 - Internet Services, 32250000-0 - Mobile phones, 32552410-4 - Modems.
8. If in the descriptions of the subject of the order there are: names of trademarks, patents or origin, this should be treated only as an aid in the description of the subject of the order. In each case, equivalent products are acceptable in terms of construction, parameters, and purpose.
9. The ordering party in accordance with the instruction of art. 29 para. 3a of the Act does not require the person / persons participating in the contract to be / were employed by the contractor or subcontractor under a contract of employment within the meaning of the provisions of the Act of 26.6.1974 - Labor Code.
