1. The subject of the contract is: successive delivery to the headquarters of the Ordering Party - PHARMACY and / or PHARMACY DEPARTMENT - (along with transport and unloading in the warehouse) pharmaceutical products listed in the price form constituting Annex 2 to the SWZ.
2. Order names and codes according to the Common Procurement Vocabulary (CPV): 33600000-6 Pharmaceuticals.
3. A detailed description of the subject of the contract and the conditions for performance shall specify:
(a) the tender form, which is Annex 1 to the SWZ,
b) the detailed scope of the subject of the contract is specified in the price form divided into sets and items, which constitutes Annex 2 to the SWZ,
(c) a draft contract, the model of which is Annex 5 to the SWZ.
4. The place of delivery shall be:
