The Achterhoek municipalities are the contracting authority and the client. The Achterhoek Social Domain provides and facilitates this purchasing procedure on behalf of the Achterhoek municipalities.
The Achterhoek Social Domain is a collaboration between the municipalities of Aalten, Berkelland, Bronckhorst, Doetinchem, Montferland, East Gelre, Oude IJsselstreek and Winterswijk.
The cooperation focuses in particular on the Social Support Act 2015 (Wmo) and the Youth Act.
The municipalities strive to achieve better and better help and support for residents of the Achterhoek. Together they achieve more than each municipality individually, is the belief.
Organizations and representatives from the broad social domain are increasingly involved in the collaboration. Such as healthcare providers, Wmo councils, etc.
The municipality of Oude IJsselstreek does not participate in the procurement of the Integrated Ambulatory Support (Youth Support) and Wmo Integrated Support (Wmo) segments.
1. The deployment of support and care is timely and aimed at an appropriate, sustainable result in a safe, familiar environment.
2. Improve the perception that your life is meaningful, based on positive health.
3. The resident participates as much as possible in society.
4. The support is flexible, aimed at cooperation and ambulantisation. Where possible, healthcare providers scale up (as quickly as possible) or downwards. If possible to the social base.
5. Youth care and Wmo providers actively cooperate in the transition from 18 to 18+ care. If care continues to be necessary, continuity is guaranteed.
