The subject of the contract is: The subject of the contract is: performance of a comprehensive service in the field of investor's supervision in the implementation of the investment task entitled: "Construction of an indoor swimming pool in the City of Otwock – stage I"

and "Construction of an indoor swimming pool in the City of Otwock – stage II" as part of the budget task entitled "Construction of an indoor swimming pool in the City of Otwock" including the preparation of complete design documentation and the execution of construction works on its basis.

3. Scope and description of the subject matter of the contract:

3.1 Scope of services provided by the Contractor:

1) at the design stage:

a) cooperation with the Designer, giving opinions and verifying the Designer's work, aimed at obtaining design documentation that will:

meet the requirements of the Polish Construction Law

contain drawings, projections, cross-sections to a sufficient extent

to confirm the feasibility of installation of devices, connections, check inter-branch coordination, location of installations,

interdisciplinary coordination,

prepared on the basis of the results of strength calculations

and states of use,

contain data on materials and construction solutions,

b) managing changes in documentation – organizing and conducting (servicing) this process

c) review of formal documents and support of the Contracting Authority in the dialogue

institutions, network administrators and administrations,

(d) familiarise themselves with the contract for the execution of works, the

and the investment area,

(e) participation in the protocol handover of the construction site to the contractor

f) preparation of a report on the verification of the executive documentation

or an updated building design prepared by the Designer (with or without comments). The Contractor will be obliged to submit the report before the contractor enters the construction site.

(2) at the stage of carrying out construction works:

a) representing the Contracting Authority at the construction site and coordinating the works being carried out,

b) control of the correctness of geodetic stakeout of the object under construction,

c) control of the proper quality of the works performed and the construction products used, including the completion of documents confirming the admission of construction products to the market in the construction industry,

(d) checking the correctness of keeping the construction diary and making entries therein stating all the circumstances relevant to the assessment of the proper performance of the construction works for the task in question,

e) control of the compliance of the performed works with the construction design and detailed designs, technical specification for the execution and acceptance of construction works, contract for construction works with the contractor, decisions, agreements and industry conditions obtained by the Contracting Authority

and technical conditions, applicable laws, applicable Polish Standards, as well as the principles of modern technical knowledge,

f) verification and evaluation of all designs prepared by the Designer

as part of the author's supervision

(g) checking the compliance of the course of works with the schedule and the timeliness of the

their performance and informing the Contracting Authority about the threat to the timely completion of works in accordance with the assumptions of the schedule,

h) supervising the safety and compliance with fire protection regulations. and occupational health and safety by all participants of the investment implementation process within the meaning of the requirements of the construction law and other applicable regulations

i) verification of deliveries of materials in cooperation with the Ordering Party,

j) reporting the progress of work along with the settlement of throughput, percentage progress of work and photographic documentation. Reports in a shortened version (information on the current status of works, photographic documentation) will be sent by the Contractor by e-mail at least

once every two weeks. At the end of each month, the Contractor will submit a detailed version, taking into account the advancement of individual disciplines, along with photographic documentation complete for a given period of work in paper and electronic form to the application office

k) promptly report to the Contracting Authority any disclosed cases of construction works performed by subcontractors not notified to the Contracting Authority by the construction contractor in accordance with the terms of the construction contract,

l) managing construction documentation (material sheets, Contractor's applications, change orders, implementation reports, notes from coordination meetings, settlement documents, etc.) and preparing notes from working meetings and sending
