The service that is the subject of the contract will be implemented on the basis of:

- The Act of 12.3.2004 on social assistance (Journal of Laws of 2018, item 1508, as amended),

- The Act of 19.8.1994 on the protection of mental health (Journal of Laws of 2016, item 546),

- Ordinance of the Minister of Social Policy of September 22, 2005 on specialist care services (Journal of Laws of 2005 No. 189, item 1598, as amended).

The subject of the contract is specialist care services for children and young people with mental disorders, provided in a place provided by the contractor in the city of Ostrów Wielkopolski or in justified cases in the apartment of beneficiaries in the city of Ostrów Wielkopolski

3. Number of hours of specialist care services for children and adolescents with mental disorders - planned number minimum 1 820 hours maximum 2 895 clock hours.

1. Teaching and developing skills necessary for independent living, including:

a) developing the ability to meet basic life needs and social functioning skills, motivating to activity, treatment and rehabilitation, conducting self-service and social skills trainings.

Interventions and help in family life

2. Providing children and young people with mental disorders access to rehabilitation and revalidation-educational classes in exceptional cases, if they are not able to access the classes referred to in art. 7 of the Act of 19.8.1994 on the protection of mental health (Journal of Laws of 2016, item 546), among others: speech therapist, psychologist, special educator depending on needs

3. Number of hours of specialist care services for children and adolescents with mental disorders - planned number minimum 1 820 hours maximum 2 895 clock hours.

4. The service should be performed in the afternoon and afternoon from Monday to Saturday, 6 days a week, excluding holidays.
