1. The subject of the contract is the provision of services in the field of providing medical transport services to patients of the Provincial Specialist Hospital in Wrocław located at 73a H. Kamieńskiego Street and 8 Poświęcka Street - hereinafter referred to as the Branch Office.
2. Transport of patients will take place:
1) on the premises of the Hospital, i.e. from the hospital's headquarters at 73a Kamieńskiego Street to the Branch Office at 8 Poświęcka Street and the Lower Silesian Center for Heart Diseases MEDINET at 73a Kamieńskiego Street,
2) within the administrative boundaries of Wrocław and outside the city limits,
3) in cases resulting from the current needs of the Ordering Party throughout the country,

2. Transport of patients will take place:
1) on the premises of the Hospital, i.e. from the hospital's headquarters at 73a Kamieńskiego Street to the Branch Office at 8 Poświęcka Street and the Lower Silesian Center for Heart Diseases MEDINET at 73a Kamieńskiego Street,
2) within the administrative boundaries of Wrocław and outside the city limits,
3) in cases resulting from the current needs of the Ordering Party throughout the country,
3. The subject of the contract includes the provision of medical transport services in the round-the-clock system on all days of the week throughout the term of the contract for Patients of the Hospital and the Branch in the scope of transport of patients requiring consultation of a specialist doctor for further treatment or consultation or diagnostic tests in other health care facilities by ambulance with a specialist team, hereinafter referred to as team S.
In addition, the Contractor will be obliged to provide the service of transporting patients of the Ordering Party with the S team in the event that such an obligation in this respect will be charged to the Ordering Party in connection with the contract between the Ordering Party and the Nfz.
