STANISŁAW SZYSZKO OF THE SILESIAN MEDICAL UNIVERSITY IN KATOWICE, according to the description in Part III of the SWZ, the tender form annex 1, the detailed offer and price form forming Annex 1a to the SWZ and Annexes No 5-8 to the SWZ and the model contract.

The subject of the order is OC insurance:

1.Compulsory liability insurance of an entity performing medicinal activity

2.Voluntary liability insurance for the conduct of medical activities

3.Voluntary liability insurance for business and property owned

in accordance with the description in Part III of the SWZ, the tender form annex 1, the detailed tender and price form forming Annex 1a to SWZ and Annexes No 5-8 to the SWZ and the model contract.

The subject of the contract is the insurance of the property:

1.Insurance property from fire and other elements

2.Insurance of glass and glass objects from bruises

3.Property insurance against theft with burglary and robbery and risk of devastation

4.Insurance of electronic equipment in the system of all risks

in accordance with the description in Part III of the SWZ, the tender form annex 1, the detailed tender and price form forming Annex 1a to SWZ and Annexes No 5-8 to the SWZ and the model contract.

The subject of the contract shall be communication insurance as described in Part III of the SWZ, the tender form set out in Annex 1, the detailed tender and price form forming Annex 1a to SWZ and Annexes No 5-8 to the SWZ and the model contract.
