The procedure is conducted in the form of an open tender pursuant to Article 132 of the PPL.The subject of the contract are the supply of tests for diagnostic tests, reagents with the lease of analyzers, test strips with the lease of glucometers according to the following parts: Task 1 - Task 11. A detailed description of the subject of the contract has been listed and described in the "Price Form" (Appendix No. 2) and in Model Agreement I (Appendix No. 3), Model Agreement II (Appendix No. 3a) and in Model Agreement III (Appendix No. 3b) constituting an integral part of the SWZ. The contractual conditions for the performance of the public contract in question contain model contracts, which are an integral part of the SWZ. In the event of grounds for entrusting the transfer of personal data, the Contractor undertakes to sign a contract for entrusting the transfer of personal data (in accordance with Annex 8 to the SWZ).
