1. The subject of the contract is the provision of services of preparation of PVC cards along with personalization and packaging for the Office of Technical Inspection.

2. A detailed description of the subject of the contract has been included in the annexes to the SWZ - Price Form constituting Annex No. 2 to the SWZ, a copy of which is attached as Annex 1 to the contract and the Draft Agreement Provisions (contract template) constituting Annex 9 to the SWZ.

3. The scope of the contract includes (applies to both parts of the contract):

a) preparation of 320,000 Sets (consisting of at least one PVC card, cover letter, packed in an envelope), including the service consisting in subprinting the card and applying the hologram to the card, printing the cover letter and their personalization and packaging,

b) packaging of 50,000 shipments consisting of at least one ready-made PVC card received from the Ordering Party, a cover letter, including the service consisting in printing the cover letter and its personalization

1) Part 1:

a) preparation of 160,000 Sets (consisting of at least one PVC card, cover letter, packed in an envelope), including the service consisting in subprinting the card and applying the hologram to the card, printing the cover letter and their personalization and packaging,

b) packaging of 25,000 shipments consisting of at least one ready-made PVC card received from the Ordering Party, a cover letter, including the service consisting in printing the cover letter and its personalization.

2) Part 2:

a) preparation of 160,000 Sets (consisting of at least one PVC card, cover letter, packed in an envelope), including the service consisting in subprinting the card and applying the hologram to the card, printing the cover letter and their personalization and packaging,

b) packaging of 25,000 shipments consisting of at least one ready-made PVC card received from the Ordering Party, a cover letter, including the service consisting in printing the cover letter and its personalization.
