1. The subject of the contract is group life insurance for employees of the Prof. S.T. Dąbrowski S.A. Hospital in Puszczykowo.

3. The Contracting Authority does not provide for the award of the contracts referred to in Article 214 (1) (7) of the Act.
4. Requirement for the lecturer or subcontractor to employ persons under a contract of employment.
1) Pursuant to Article 95(1) of the Act, the Contracting Authority requires that persons performing activities in the scope of the implementation of the subject of the contract, the performance of which consists in performing work in the manner specified in Article 22 § 1 of the Act of 26 June 1974 – Labour Code (i.e. Journal of Laws of 2020, item 1320, as amended), be employed by a contractor or subcontractor on the basis of an employment contract.
2) The type of activities necessary for the performance of the contract, which are subject to the requirements of employment under the employment contract by the contractor or subcontractor of persons performing activities during the performance of the contract and the Method of verification of the employment of these persons and the contracting authority's right to control the contractor's compliance with the requirements related to the employment of these persons and sanctions for non-compliance with these requirements are presented in the draft contract.
4. The Ordering Party does not guarantee joining the insurance of a certain number of people, but indicates that it will complete the order at a minimum of 50%.
5. The Agreement shall be valid for a maximum of 40 months from the date of its conclusion, which shall be divided into the following periods:
1) the so-called implementation period – up to 4 months,
2) the period of insurance protection (relevant insurance contract) – 36 months.
