The subject of the contract is the performance of cleaning and auxiliary activities, being in connection with medical services, in hospital wards and medical and administrative rooms of MSCZ with its registered office in Pruszków at 2/4 Partyzantów Street. A detailed description of the subject of the contract is Annex. No. 2 to SWZ – Description of the Subject of the Contract, together with the following. attachments: Annex. No 2.1 – List of working hours; Incl. No 2.2 – Inspection report; Incl. No. 2.3 – Cleaning procedure. The subject of the contract is performed in hospital wards, diagnostic rooms and administrative rooms.
1. The subject of the contract is the performance of cleaning and auxiliary activities, being in connection with medical services, in hospital wards and medical and administrative rooms of MSCZ with its registered office in Pruszków at 2/4 Partyzantów Street. A detailed description of the subject of the contract is Annex. No. 2 to SWZ – Description of the Subject of the Contract, together with the following attachments: Appendix. No 2.1 – List of working hours; Incl. No 2.2 – Inspection report; Incl. No. 2.3 – Cleaning procedure.
2. The subject of the contract is performed in the following departments and medical and administrative rooms: 1) hospital departments: Department IAB, IC, II DE, IV, VI, VIIA, X, XI with the Admission Room, XIV, ZOL VIII A and B, ZOL XII, ZOL XV, Day Ward, Addiction Therapy Clinic, Court Section, Day Addiction Therapy Department, Mental Health Center (CZP) 1 and Mental Health Center (CZP) 2 in another location (Grodzisk Mazowiecki, ul, Składowa 5); 2) diagnostic rooms: X-ray (X-ray), Magnetic Resonance Imaging (MRI), Consultants' Offices, Point of Collection of Test Materials, Laboratory of Electroencephalography (EEG); 3) administrative rooms: the building of the Logistics Department, the building of the Directorate (second floor), the Palace over Loss.
3. The subject of the contract is carried out around the clock, in the form of a maximum of 24-hour duty.
4. The list of man-hours in individual organizational units of the Ordering Party is Annex. No. 2.1 to SWZ. The Contracting Authority reserves that the number of hours will result from its current needs and may differ from the one given, whereby the monthly flat rate is fixed, and each subsequent hour will be additionally settled at the rate provided by the Contractor in the Offer Form, constituting Appendix. No. 1 to SWZ
