Wolverhampton CCG are seeking a single provider to deliver a medically led community dermatology service. The successful provider will deliver a high quality and patient centred service for all adults with common dermatological conditions who are registered with a Wolverhampton GP Practice.

The services to be delivered are:

Clinical Assessment, investigation and treatment

See and treat model of care where appropriate

Provision of care plans for GPs

Follow up management (New to review follow up ration of 1:2)

Education sessions for local GPs

Reduced waiting times for treatment

Advice, treatment and support for self management of chronic dermatological conditions.
