6.1.Each contractor must be joined by:

6.1.1. in order to confirm that the deliveries offered comply with the requirements laid down by the Contracting Authority, a completed technical specification, annex 13-21 to this SWZ, must be submitted to the tender.

6.1.2. power of attorney in the form specified in clasps 9.12-9.15 SWZ

6.1.3. in order to confirm that the products offered are medical devices, the following shall be accompanied by:

(a) Current declarations of conformity or certificates of conformity or notification/notification to the President of the Office for Registration of Medicinal Products, Medical Devices and Biocidal Products, in accordance with the Medical Devices Act of 20 May 2010. /t.j. Journal of Laws of 2020, item 186 of col./ - applies to parts no 1, 7, 8 and 9

(b) Contractor's declaration that the products offered are entered in the list of Biocidal Products maintained by the Office for Registration of Medicinal Products, Medical Devices and Biocidal Products in accordance with the Biocidal Products Act of 9 October 2015 (Journal of Laws of 2018 item 2231 as amended) — Applies to Part No 3

6.1.4. Completed price form according to the formula set out in Annex 4-12 to SWZ — according to the part offered.

6.1.5. The Contracting Authority shall request the submission with the offer of the evidence given in paragraph 6.1.1, 6.1.3 to confirm the conformity of the supply offered with the requirements set out in the description of the subject of the contract.

The Contracting Authority shall accept the equivalent measures of evidence in question if they confirm that the deliveries offered meet the requirements laid down by the Contracting Authority.
