The delivery of long term home support, specifically for the Sherbourne, Whoberley and Earlsdon suburbs of Coventry, for Adults and Children that meet national eligibility criteria for social care and adults eligible for non-complex health funded Continuing Health Care at home. Support will be for eligible adults and children / young people and will provide personalised care and support to people in their own homes to enable individuals to live independently. The service will also assist carers / parents to maintain their caring role.
