GROUP LIFE AND HEALTH INSURANCE SERVICE OF WORKERS AND SOLDIERS AND THEIR FAMILY MEMBERS 13 MILITARY ECONOMIC BRANCH AND SUPPLY UNITS - 68/2021

The subject of the order is group insurance services for employees and soldiers and members of their families 13 WOG and stocked units, as follows:

Task No 1: Group life insurance for employees and servicemen and their family members 13 WOGs and supplied units, in particular insured death or mishap benefits, family benefits and condition benefits Disease.

Description of the item by CPV code: 66511000-5, 66512000-2.

Task No 2: Group health insurance for employees and servicemen and members of their families 13 WOGs and supplied units, in particular: specialist consultations, laboratory and diagnostic examinations, home visits, outpatient procedures. Description of the item by CPV code: 66512210-7, 85120000-6.

Group life insurance for employees and servicemen and their family members 13 WOG and stocked units, in particular death or mishap benefits

The subject of the contract is group life insurance services for employees and soldiers and members of their families 13 WOG and stocked units, in particular the insured's death or mishap benefits, family benefits and benefits related to onset of a pathological condition.

The Contracting Authority does not guarantee that all employees, soldiers and their family members will join group life insurance. The binding of the insurance contract will be conditional on the individual decision of each employee/soldier/family member.

During the duration of the contract, the amount of insured may change.

The Contracting Authority stipulates that the insurance is voluntary and if the employee or soldier or family member of the employee resigns from the insurance, the Ordering Party is not obliged to pay the insurance premium for them.

The contract is provided for to be financed by own funds of employees and soldiers of the Ordering Party and supplied units. The unit cost of the insurance service for one reported person will be borne by an employee/soldier.

The Contracting Authority shall only make payments in this respect by making monthly deductions from salaries or other payments of employees or soldiers and transferring the contribution to the Contractor's account.

The Contracting Authority shall not make deductions from work remuneration and transmit insurance premiums to other entities, including the Insurance Companies and other insurance intermediaries, than the Society established in the course of this proceeding.

The Contractor shall provide the operation of the insured at the headquarters of the Military Units defined in the SWZ in a manner considered to be the most optimal, e.g. by its employees or otherwise chosen by it, ensuring continuity of insurance coverage and timely payment of benefits.

The Contracting Authority reserves, pursuant to Article 121 of the PZP Act, the obligation to personally perform insurance services falling within the scope of the contract, in particular as regards the collection of insurance premiums, guarantee to insured benefits by payment of contracted cash sums or benefits in if the occurrence provided for in the contract of the event in the life of the insured person.
